by the policyowner and returned to us. Please note that no action
will be taken on a specific request until a properly completed
form is received and accepted by us. By providing these forms
we do not confirm that coverage is in force or that your request
can be processed.

life insurance claims please be sure to include a certified copy
of the death certificate along with a completed claim form. However,
before completing a claim form we recommend that you call us first
so that we may verify the claim requirements.
